Obidoxime

Obidoxime chloride solution for injection 250 mg/amp

中毒物質：

有機磷殺蟲劑 (parathion、mevinphos、methomidaphos、dimethoate 、dichlorovos 、trichlorophos、chlorpyrifos )、生化武器VX、沙林毒氣等中毒之解毒劑

使用劑量：

成人劑量：

Loading dose: 1 ample (250 mg) slow infusion intravenously

Maintenance dose: 750 mg/24 h continuous infusion

兒童劑量：

Loading dose: 4–8 mg/kg, slow infusion intravenously.

Maintenance dose: 10 mg/kg BW/24 h continuous infusion.

注意事項：

  1. 與250 ml 5%葡萄糖或250 ml生理鹽水相容，持續24小時。
  2. 可能可以治療nicotinic effect。使用後應收集全血2 ml 以檢測RBC及plasma cholinesterase活性。
  3. 不建議使用於氨基甲酸鹽類殺蟲劑中毒病患，因為可能無效，甚至可能增加氨基甲酸鹽的作用。
  4. 常見副作用：在健康受試者中，症狀在2小時內自行緩解，標有*的為中毒病患中觀察到的可能副作用：dysgeusia (taste of menthol), hypoaesthesia, cardiac arrhythmia*, dry mouth, jaundice cholestatic* (with doses exceeding 3000-10000 mg within 1-3 days), hepatic function abnormal* (with doses exceeding 2000 mg), muscular weakness, feeling hot, feeling cold, heart rate increased, blood pressure increased, electrocardiogram change
  5. Obidoxime不會與血漿蛋白結合，Vd約0.171~ 0.32 L/kg，主要通過腎臟排泄。半衰期約為2小時，原形物質隨尿液排出。給予藥物2小時後排出52%，8小時後排出87%的注射劑量。
  6. IV給予obidoxime在血漿濃度達到3.6-7.2 mg/L，可預期顯著的重新活化膽鹼酯酶。在高濃度下，obidoxime本身會輕微抑制膽鹼酯酶，不過在建議劑量下不會產生此作用。
